fix: 修复禅道Bug #330 门诊医生站诊断保存失败问题

- 修改前端日期格式，从ISO格式改为 yyyy/M/d HH:mm:ss 格式
- 添加后端参数校验，防止NPE异常
- 优化前端错误提示，显示后端返回的具体错误信息
