Fix Bug #552: 双击待保存医嘱编辑保存后不应自动添加空医嘱 — 根因：handleSaveSign 中自动添加空行的条件缺少 isAdding.value 判断，导致双击编辑已有待保存医嘱也会触发 handleAddPrescription()
