Fix Bug #468: 住院医生工作站-检验申请列表页缺失单据状态列

在检验申请列表页"申请单号"后新增"单据状态"列，显示单据生命周期状态
（待签发/已签发/已采集/已收样/报告已出/已作废），使医生可通过列表直接获知
申请单当前执行阶段。
