feat(传染病报卡): 新增传染病报卡管理功能模块

实现传染病报卡的基础功能，包括：
1. 新增报卡查询参数DTO、报卡详情DTO和状态枚举
2. 添加报卡Mapper接口及XML实现分页查询和详情查询
3. 实现报卡AppService接口及Controller提供REST API
4. 新增前端API接口定义
5. 添加审核记录实体类
