Fix Bug #472: 住院医生工作站-手术申请单：勾选手术项目无效，导致无法正常开立医嘱

surgery.vue 中 ref 和 watch 未从 vue 导入，导致 transferValue 不是响应式变量，
el-transfer 勾选复选框时状态无法更新，修复 import 语句补充 ref、watch。
