chore(router): 添加患者档案管理路由配置

- 添加患者档案管理路由配置代码
- 注释掉租户用户设置路由部分代码
- 新增patientmgr路由项配置
- 配置路由组件和元信息
- 设置路由隐藏属性
- 完善路由路径和名称定义
